After years of separation, quadruplets GIRLIE the girl, PETER the boy, MARK the gay, and PANYING the lesbian (all played by Vice Ganda) meet again when Peter needs a compatible liver donor. Among them, Mark is the only match.
But Mark does not give his liver so easily, bearing years of resentment towards his father Pete (Joey Marquez) and his other siblings. He will only agree to donate if they experience the hardships he, Panying, and their mother Pia (Maricel Soriano) have gone through for having been neglected as family.

In the call center, she meets people coming from different walks of life; fun-loving Richie (John Lapus), young -living Lolay (K Brosas), prim and proper Martin (Alex Castro), and their monster Team Leader Vince (Enchong Dee).
Disastrous as it may be, little does she know that more than the money and the affection of her daughter, she will learn more important things inside the call center; the creation of a new family outside her own and a new sense of purpose in life.

If they're lucky, these actors can stand beside the movie star in a scene, but we don't really notice them. Usually, only half of their face is seen--and blurred at that. We won't really call them actors; after all, we know actors by their names, right? They are just bit players, or extras, as they are called in the Philippines. They have been nameless for a long time, and today, we finally get one of their names. LOIDA MALABANAN, 50's, a professional extra. If you ask her though, she's an actress.
Extra follows a seemingly usual day in the life of loida as she embarks on yet another shooting day as an extra. From looking for a black cocktail dress at 2am, sleeping at 3am only to wake up at 3:30am for a 4am calltime, to getting a role as WORKER 1, as a maid with dialoques and a role of a lifetime, we see that while an extra is usually overlooked in a scene, they have lives too. And sometimes, their lives are even more cinematic than the film scenes they are part of. They don't realize this though; for them, all's just in a day's work. Unknown to LOIDA and her colleagues is the truth being tightened in the ruling system of production, the exploitation on the marginalized laborers like them as great extras. For us though, we will know they are more than just extras. In fact, they could actually be bigger than life.
